ESWT, known in Turkish as shock wave therapy, is a new generation treatment method in which ultrasonic sound waves created outside the body are converted into therapeutic level energy and applied to the affected body area in the form of shock waves through a special applicator without disrupting tissue integrity. Microtraumas are created that stimulate blood vessels and surrounding stem and mature cells at the tissue level, enhancing healing and regeneration.
Currently, and based on ongoing studies, ESWT is used in three main conditions in urology:
Erectile Dysfunction: A condition characterized by the inability to achieve or maintain an erection, often associated with hormonal and anatomical issues.
Peyronie's Disease: A progressive disease that causes curvature, painful erections, and penile deformity due to fibrotic plaques in the penis.
Chronic Prostatitis: Known in modern classification as "Chronic Pelvic Pain Syndrome," this condition is characterized by pain in the genital area, intermittent urination, frequent urination, and burning during urination. It is often confused with BPH (benign prostatic hyperplasia), leading to delayed diagnosis.
ESWT is currently listed as a first-line treatment in the guidelines of the European Association of Urology.
Microtraumas created by ultrasonic shock waves activate mature and stem cells responsible for forming new blood vessels and tissues in the affected area, increasing blood flow and achieving permanent tissue-level healing. Blood vessels, which are hormone-secreting organs with their own pressure sensors, form natural bypasses to protect their structure when abnormalities occur. ESWT shock waves highly activate this ability in vessels and tissues that cannot renew themselves.
ESWT provides a new treatment protocol not only in urology but also in other medical fields for chronic and difficult-to-treat conditions. It is used in orthopedics for frozen shoulder, heel spurs, and synovitis; in physical therapy for dorsalgia and other painful muscle diseases; and in cardiology for coronary diseases.
One course of ESWT treatment consists of 6 sessions. (In severe cases of erectile dysfunction and Peyronie's disease, at least 12 sessions are recommended in the global literature.) The treatment protocol, coordinated between the doctor and patient, is usually completed with 2 sessions per week over 3 weeks. Alternatively, sessions can be completed daily within 15-20 minutes each. Hospitalization and anesthesia are not required. The procedure is painless and involves external regional treatment of the genital area using an ultrasonic applicator.
With ESWT treatment, patients can undergo outpatient treatment without hospitalization and continue their daily activities without any issues after the sessions.
The greatest advantage for patient compliance is that it is a painless and providing new generation treatment which consist of no pain. Studies have shown no reported significant side effects to date.
ESWT has also been shown to be more successful than all other treatment protocols in these areas.
